QABA Monthly & Final — Fieldwork Documentation Guide
The monthly log is an official QABA supervision record completed at least once a month; the final verification form rolls those totals up and is submitted once — with supervisor signatures — at application. Below covers how to fill the monthly log (QBA·QASP-S) and the final forms (QBA·QASP-S·ABAT), with samples.
The monthly log should be closed out each month-end
The log should be confirmed and signed by your supervisor at each month-end — ethically and by the rules. If left blank, filling it in retroactively may not be accepted. From 2026-09-01, QABA also requires a signed Fieldwork Documentation Form before exam approval.
1. Where the monthly log fits
- ① Session logs — your day-to-day session records.
- ② Monthly supervision log — the month rolled up into hours and supervision ratio — the form this guide covers.
- ③ Final Verification Form — the final signed summary; totals flow up from ②.
Terminology — “Independent hours”
Columns A·B are “Independent Hours (supervisor not present)” on the form — hours worked without a supervisor. They are distinct from C·D (supervised hours).
2. Samples by credential
The QBA and QASP-S forms are different documents. QBA has a dated session-log table filled row by row; QASP-S has no session log and records totals for the supervision period instead. The requirements also differ in total hours and in the direction of the 60% indirect rule. The supervisor shown in each sample is fictional. ABAT has no monthly log — see the final forms in section 7 below.

3. How to fill each item (QBA session log)
The table below describes the QBA session log. A (direct) vs B (indirect) depends on whether the work is with the client; both are entered as timeframes.

QASP-S is filled in differently
The QASP-S form has no dated session table. You enter the supervision period, then total that period's hours on five lines: A) direct implementation, B) indirect activities, C) individual supervision, C) small-group supervision, D) the sum of A through C. Supervisor meeting time is excluded from A and B. You then mark the meeting method (individual / group / group size), check the activities covered, pick one of three competency ratings, and both supervisee and supervisor sign. Keep your own day-to-day session records separately; only the totals go on the submitted form.
4. Supervision ratio
Percent of Hours Supervised
Supervised (C+D) ÷ Independent (A+B) × 100 must be ≥5%.
- 100 independent hours → at least 5h supervision (5 ÷ 100 = 5.0%).
- 130 independent hours → 6.5h minimum; the sample records 7h (7 ÷ 130 = 5.4%).
- 5% is a floor — round up so you don't fall short by a decimal.
- At least one contact must be in-person or live video for ≥1h. Group counts up to 50% of supervision.
5. When you run out of rows (QBA)
- Use additional pages if one isn't enough — at least one page per month.
- Multiple sessions in a day can share a cell with several timeframes.
- QASP-S has no session table, so this does not apply — you record totals for the supervision period.
- Log as you go and keep records for 7 years. Only ink or electronic signatures are accepted.

7. Final verification forms
Once your monthly logs are complete, the final verification form rolls those totals up and is submitted once — with supervisor signatures — at application. QBA·QASP-S summarize the monthly totals; ABAT has no monthly log and is completed with this single form. Always submit using the official QABA form.
