⚠ Not an Official QABA Form — A Helper Tool
This fieldwork system is a helper tool for daily record-keeping, progress visualization, and drafting internal reports. Under QABA Code of Ethics §7.7 and §9.4, trainees and supervisors are required to complete the official supervision documents issued by QABA, have both parties sign them, and retain them for at least 7 years. This system does not automatically generate signatures or PDF evidence, and its records alone are not accepted as proof when applying for or reviewing credentials.
